WITHDRAWAL CARDS

Be sure to request a Withdrawal Card when being laid-off, going on leave of absence, lengthy medical leave or terminating your employment.  The charge of a withdrawal card is 50 cents and can be obtained through the Local Union dues office. All initiation fees and dues must be up to date in order for a withdrawal card to be issued.  It is your responsibility to obtain a withdrawal card as soon as possible after leaving the company.

Failure to request a withdrawal card may cause you to pay back dues.  Any questions regarding withdrawal cards or any other dues inquiry can be directed to Lexie Cheatham in the dues office at Local 651.

If you are having problems,need a form or have questions regarding your Central States Pension/Insurance, please contact Sara Clark at the Union Hall.
